Itchy skin rash at site of squamous surgery

My husband has a 7 inch scar on his shin where he had surgery 6 months ago for squamous skin cancer. About 3 months post surgery, he developed an itchy skin rash in the area consisting of small “bumps” under the skin and a general swollen look to that area.

As time goes on, this “rash” has slowly migrated away from either side of the vertical suture line and is proceeding towards the calf of the leg. Also about the same time this rash originally appeared, it also appeared on his elbows. Now he has a rash on his upper back too ! Been to dermatologist twice and family dr. once. Nothing they prescribe work.

Question: What, if any, relationship might this rash have to do with the surgical procedure. I know there were internal stitches. Any help is appreciated. The itching is awful to put up with !!!
